Billing

  • If you do not have insurance, or are recently unemployed and no longer covered by insurance, you may qualify for financial assistance. Even if you have insurance, financial assistance may be available under certain circumstances. A summary of the Montefiore financial assistance information can be found on their Financial Assistance page.

  • If you have questions about an invoice or statement you have received related to your services, contact your MOTION clinic, or email us at [email protected].

  • You may pay your invoice by check or major credit card in person, by mail or online through MyChart.

  • You can request Itemized statements by emailing [email protected].

    Note: If you send health information to MOTION via email, please encrypt your email by including “[secure]” in the subject line.. An unencrypted email means there is a risk that the information in the email and any attachments could potentially be read by a third party when it is sent through the internet.

  • Before your visit at MOTION, confirm the following with your insurance company:

    1. Is my insurance accepted by MOTION Sports Medicine?
    2. Which services are (and are not) covered under my plan?
    3. Do any of these services requires authorization?
    4. “Do I have a copay? If so, what is the amount for each visit?”Do I have an unmet deductible amount?
    5. What are my coinsurance and out-of-pocket expenses?
  • After your visit, a claim will be generated summarizing the services you received and sent to your insurance provider. Your insurance will then notify MOTION if you are responsible for a portion of the provided services. Your insurance will usually identify any patient responsibility by checking the following items in your benefits plan:

    • Deductible: The amount you pay for covered health care services before your insurance plan starts to pay.
    • Coinsurance: The percentage of costs of a covered health care service you pay (20%, for example) after you’ve paid your deductible. The percentage is based on your specific plan you have chosen.

    You will receive a billing statement only after your services are processed by your insurance provider for each date of service.

  • The time it takes to receive an invoice varies depending on your health plan, usually anywhere from 2-4 weeks. Your health plan determines if you are financially responsible for a portion of services based on your deductible and coverage. Your insurance company should provide an Explanation of Benefits informing you of claims submitted, how much will be paid by the insurance company, and how much you will owe. Once your insurance company has processed the information, you will receive an invoice.

    You will receive your bills in the mail, and they will also be accessible via MyChart.

  • Your financial responsibilities related to your plan of care will depend on your specific health plan (insurance). At your first appointment, we will review with you what we know about your health plan, to help you understand and anticipate your financial responsibilities.

  • Co-pays are due at the time of service, per your insurance plan.

    If an estimate was provided prior to your visit, a portion of that amount may be requested at the time of your visit. If you have any outstanding balances, you may be asked to pay your balance or make payment arrangements.

    1. Look for your insurance plan on our accepted insurance page.
    2. A scheduler can check if your insurance is accepted when you call to schedule your visit.

  • Yes, many times they do. In fact, we will verify your benefits for you. When you walk in for your first visit, your benefits will be explained to you thoroughly. This service is complimentary to you, and the purpose is that there will be no mystery surrounding your benefits.
