How is the amount determined?

After your visit, a claim will be generated summarizing the services you received and sent to your insurance provider. Your insurance will then notify MOTION if you are responsible for a portion of the provided services. Your insurance will usually identify any patient responsibility by checking the following items in your benefits plan:

• Deductible: The amount you pay for covered health care services before your insurance plan starts to pay.
• Coinsurance: The percentage of costs of a covered health care service you pay (20%, for example) after you’ve paid your deductible. The percentage is based on your specific plan you have chosen.

You will receive a billing statement only after your services are processed by your insurance provider for each date of service.
