Before your visit at MOTION, confirm the following with your insurance company:
- Is my insurance accepted by MOTION Sports Medicine?
- Which services are (and are not) covered under my plan?
- Do any of these services requires authorization?
- “Do I have a copay? If so, what is the amount for each visit?”Do I have an unmet deductible amount?
- What are my coinsurance and out-of-pocket expenses?
